DISEASES OF THE SKIN : PARONYCHIA AND ONYCHIA YEAST (PARONYCHIA ET ONYCHIA BLASTOMYCETICA) - etiology, pathogenesis, symptoms and course of, recognition, prediction and prevention
The disease is caused by yeast-like fungi of the genus Candida (Monilia). Among the predisposing factors prevail, trauma and maceration of the skin. In some cases, the leading role belongs to disturbances of metabolism, especially carbohydrate. Sources of infection: yeast-like fungi, vegetative fruits and vegetables, as well as the products of their processing; yeast-like fungi from the gut (autoinfection); possible contamination during a manicure using tools. Ill more often than women.
Symptoms. Pincushion paronychia: the cushion as if hanging over the Lune nail, skin, covering, shiny, sharply hyperemic, on the edge of the cushion silvery-white scales; nail skin (eponychion) vanishes. The lateral edge of the nail plate straight, as if cut, soft consistency, brokeriai color. The surface of the plate has a transverse furrow.
The course is chronic with periodic exacerbations paronychia. Subjectively during exacerbations observed shooting pain. There are cases of self-healing.
Recognition is based on the presence of typical pincushion paronychia, the lack of nail skin and the characteristic change of the side edges of the nail plate. Microscopic examination of scrapings from the side edges of the nail, and flakes from the edge of the cushion elements found yeast-like fungus: a handful of yeast cells and short thin strands of mycelium.
Treatment mostly, as in ringworm of the nails (see ringworm); mandatory treatment of paronychia. On the entire nail phalanx re-impose a compress bandage ointment from Arievich (see ringworm), after which the softened side edges of the plate srisawat scissors; simultaneously removes the Horny layer of the skin covering the cushions. In the further blurring of 2% iodine tincture and 10% aqueous solution polutorakratnogo iron alternately within 2 to 3 weeks. When persistent paronychia recommended daily hot baths and a pellet of pure Ichthyol (as in the treatment of furuncle see).
