EYE DISEASES : DETACHMENT OF THE RETINA (АBLATIO RETINAE) - etiology, pathogenesis, symptoms and course of, recognition, prediction and prevention
Etiology. Degeneration of the retina and the vitreous body on the grounds of high myopia, senile changes, injuries.
The pathogenesis. Gap degenerated retina due to tension hems formed in the vitreous body; the subsequent development of subretinal exudate and penetration through the gap under the retina the vitreous fluid.
Symptoms. Muddy, gathered in folds, with the convoluted and very dark vessels the retina will hold one or more bumps in the vitreous. Subjective - first flicker of sparks and fiery, zigzags, etc., then the darkening of the field of view from any side.
For. The gradual increase of the detachment. Later comes the degeneration detached retina and then becomes irreversible blindness. In this stage of detachment is usually complicated by the development of complicated cataract.
Simple detachment differs from detachments caused by tumors of the choroid and retina , see Glioma (neuroblastoma) gridshells, folding, absence from the depths translucent grayish tubercles, on which (in tumors) the retina appears to be tightly stretched and does not fluctuate during eye movements, the presence of breaks though. The final diagnosis is based on the transillumination x-ray lamp of the legislative Assembly or Lange. Simple, essential detachment does not interfere with the fluorescence transillumination of the pupil.
Prevention is to respect a very calm mode Miami high and medium degrees. Avoid sudden movements, heavy lifting. Good correction of myopia glasses.
Treatment with surgery alone. Perhaps more important early surgery, within the first few months after the occurrence of detachment. Then operation gives 60 - 70% success rate. In later operations the visual success becomes questionable even with good adhesion peeling of retinal membranes. Therapeutic measures (rest, compressive bandage, injection of hypertonic salt solution under the conjunctiva) is now almost completely abandoned.
