EYE DISEASES : ALLERGIC TUBERCULOUS KERATO-CONJUNCTIVITIS (КЕRАТО CONJUNCTIVITIS TUBERCULOSA ALLERGICA S. SCRO - etiology, pathogenesis, symptoms and course of, recognition, prediction and prevention
Etiology. Specific hypersensitivity of the body, due to tubercle bacilli.
The pathogenesis. Hyperergic state of the external membranes of the eye on the soil of tuberculous intoxication. The first manifestations of diseases of the eye often occur in children with tuberculous primary complex. In later periods the process in the eye can be triggered in children suffering from scrofula, any non-specific local impacts banal conjunctivitis, superficial injury of eye, eye fatigue enhanced visual work, bad treatment of the existing refractive and muscular anomalies. It occurs mainly in childhood and early adolescence.
Symptoms. Heavy subjective sensations - a feeling of burning, photophobia, lacrimation, blepharospasm. Sharp hyperemia of the mucous membranes of the eyelids and especially the eyeball. On the mucous membrane of the eyeball rash of small nodules, varying in size from a poppy to a millet seed, a so-called conflict. The conflict is located mainly around the limb. Sometimes moving a limb on a transparent corneal tissue, giving a picture phlyctenular keratitis. In older and neglected cases, there is a heavy process in the cornea, resulting in significant reduction of view, the phenomenon of keratitis fasciculare (see), or even TFR - polesny the pannus.
For prolonged, chronic, with separate exacerbations. Often, upon reaching sexual maturity all phenomena spontaneously subside. In patients with significant corneal changes can be extensive scarring of the cornea, greatly decreasing vision (corneal opacity).
Recognition usually is not difficult. Children almost always there are other manifestations of tuberculosis of the glands. Cervical cancer usually condensed, often Zhasulan; there are fistulas. Very often there is tuberculosis of the skin of the face and scalp, suppuration of the ears. Tuberculin reaction (pirok, Mantoux) as a rule, sharply positive.
Rational diet without overloading on carbohydrates, rich in vitamins diet, mainly dairy vegetable, exposure to air at least 5 - hours a day, a routine day (for students), visual hygiene in school and at home, complete correction of refractive errors.
Fish oil inside. Calcium preparations. Phytin. The overall irradiation with ultraviolet rays (in the absence of active intrathoracic specific process). Galvanic collars with calcium on Shcherbak. Topically calcium-instores, yellow mercury ointment. In the presence of corneal changes - insert corneal powder of calomel (just the doctor). With active corneal process with a strong reaction from the iris prescribe atropine. During the formation of marginal ulcers of the cornea shown pilocarpine. In cases of associated infectious process in the conjunctiva - sulfacetamide topically in the form of drops or penicillin by rapid method (see acute Conjunctivitis).
